Also, does it bug anyone else when the staff fills the to-go box? I don't want to have to explain what I do and don't want to take home, and it's just awkward to have your food taken out of sight and then brought back, despite it having originally been prepared out of sight.

This is quite rare in the UK.

I was shocked at how common it was when I started visiting the US on business, including ordering stuff on expenses and barely touching it at the restaurant.

And it's common to do so.

So what's exactly the big insight?

Most, if not all, of the restaurants I've been globally also "refrain from encouranging". But all of them when asked would bring a "doggie bag" or package to take the food home...
